for submission to the Registry Office. Submissions can be

| Application to Graduate |
This application must be submitted to the Registry
Office by November 15th of the academic year in which
you intend to graduate. There is no fee for an application
submitted by this deadline. Applications received after
November 15th must be accompanied by a $50 late fee. |

| Certificate
of Specialization in Organic Agriculture |
To apply for a Certificate of Specialization in Organic
Agriculture please submit this completed form to the
Registry Office. To be eligible for this certificate
you must have completed four eligible organic agriculture
credit courses, at least two of which must have been
completed at NSAC. An overall average of 60% in the
courses is required. |

| Confirmation of Enrollment |
To confirm your enrollment in an NSAC program, this
form must be completed and submitted to the Registry
Office. Confirmation of enrollment for student loan
purposes requires a separate form and/or a transcript
or your marks. |
|
|
| Incomplete Grade Request |
Students are expected to complete class work by the prescribed deadlines. A student who fails to complete the required work for a particular course during the established time period of the semester can expect to receive a failing grade. However, where exceptional circumstances interfere in the completion of all the requirements necessary to pass the course such as acute illness, family emergency, death of a close relative, the student may apply to receive a grade of "Incomplete" (IN) in the course. |

| Intention to Graduate
in Absentia |
If you are unable to attend the convocation ceremony
in which you are expected to graduate, you must submit
this form to the Registry office by April 30th. |

| Letter of Permission |
If you intend to take a course at another university
for credit transfer to NSAC, you must obtain permission
from the NSAC Registrar in advance. Please submit this
completed form for review. |
|
|
| Letter of Permission - International |
If you intend to take a course at an international
institution for credit transfer to NSAC, you must obtain
permission from the NSAC Registrar in advance. Together
with your Exchange Program Coordinator and NSAC Program
Advisor, complete and submit this form for review. |

|
| Notification of Drop/Fail |
If you wish to withdraw from a course after the deadline
to drop a course and before the last day permitted to
Drop Fail a course, you must submit this completed from
to the Registry Office. The last day to drop a course
without academic penalty is the Friday of the seventh
week of classes. The deadline to Drop Fail a course
is the Friday of the eleventh day of classes. For detailed

| Notification of Program Change |
To declare or change your program, major, or minor,
please submit this completed form to the Registry Office. |
|
|
| Permission to Challenge
for Credit |
Students who have acquired competence in material
covered by an NSAC course may obtain credit for the
course by means of a course challenge. The procedure
for challenging for course credit can be found in the
Academic
Calendar. |

|
| Registration Amendment |
To make changes to your course load that you are otherwise
unable to make using the WebAdvisor Course Registration
system, please indicate the desired amendments on this
form and submit the completed form to the Registry Office.
Circumstances where this form may be needed include
registration for No Program students, timetable conflicts,
and to approve an overload of courses (i.e. 7 degree
level courses or 8 technical level courses). |

| Student Information
Change |
The Registry Office appreciates having current phone
numbers and local and permanent addresses for each student.
Students are asked to keep their contact information
up to date by submitting any changes occuring during
their study at NSAC. |

|
| Student Withdrawal |
Students who are considering withdrawing from the
NSAC should first consult with their advisor and the
Registrar. Anyone who wants to withdraw after meeting
with their advisor and the Registrar must submit the
following form to the Registry Office, along with their
Student ID card. |

|
Third Party Release of Information
|
NSAC is committed to protecting the privacy of student academic records and personal information. However, students can elect to extend access to their academic record, should it be required, to a third party. By submitting a signed ''Third Party Release of Information' form, students may authorize the Registrar's Office, in advance, to release academic information to any person(s) or organization(s) they specify. This form will be accepted by mail, fax or in person. All submissions must be accompanied by either a copy of the student's driver's license, passport or student card.

| Transcript Request |
If you require a copy of your undergraduate transcript,
please complete and submit this form by mail, fax, or
in person. There is no charge for transcripts. Graduate
transcripts must be requested through Dalhousie University. |

|
| Tuition Waiver |
NSAC faculty and staff, and their spouses and children,
must submit this form in order to apply for a tuition
waiver. NSAC faculty and staff are entitled to enrol
in one credit course per calendar year on a tuition-free
basis. Children and spouses of NSAC employees will be
entitled to enrol in undergraduate and technical courses
at 50% of the normal tuition costs, provided they are
enrolled as full-time students and meet admission requirements. |
